Time to Update Your Strength Training Regimen?

Strength training, also called resistance training, is an important method for controlling blood sugars without drugs in folks with diabetes.

A few months ago I read Hillfit: Stength, an ebook  by Chris Highcock of Conditioning Research.  It’s about  improvement of hiking skills and enjoyment via strength training with without having to join a gym or buy lots of equipment.  I’ve been on Chris’s program for the last five weeks.

One of the scientific review articles he cites in support of his recommendations is an eye-opener.  Evidence-Based Resistance Training Recommendations is available free online.  It’s published in Medicina Sportiva, which I’m not familiar with.  I’ll confess I’ve read little of the hard-core literature on the science of strength training.  It’s one of my more recent interests.

An excerpt:

We recommend that appreciably the same muscular strength and endurance adaptations can be attained by performing a single set of ~8-12 repetitions to momentary muscular failure, at a repetition duration that maintains muscular tension throughout the entire range of motion, for most major muscle groups once or twice each week. All resistance types (e.g. free-weights, resistance machines, bodyweight, etc.) show potential for increases in strength, with no significant difference between them, although resistance machines appear to pose a lower risk of injury.

The article has got me questioning some of my long-held notions, such as how often to work out, number of reps moving a weight, speed of moving a weight, and whether I should stick with the free weights I tend to prefer.  Why not see if your dogma is supported?  Worth a look.

Review of Chris Highcock’s Hillfit

Chris Highcock over at Conditioning Research has just released a new ebook on strength training for hikers: Hillfit: Strength.  Hiking is one of my favorite hobbies.  I particularly like walking up hills and mountains.  If you’re ready to reap the benefits of resistance training, this jargon-free plan is an excellent starting point, and may be all you’ll ever need.  Even if you never go hiking.

Chris is a fitness columnist for “TGO (The Great Outdoors).”  He lives and hikes in Scotland.  Chris’s goal with the program is to increase your enjoyment of hiking by increasing your level of fitness. 

He clearly presents four basic home exercises requiring no special equipment; they’re bodyweight exercises.  You get it done in 15 minutes twice a week!  The key is to do one set of each exercise, slowly, to exhaustion.  What’s slow?  Ten seconds for both lift and lowering.  For instance, when you do the push-up, you push up over  the course of 10 seconds, then let your body down slowly over 10 seconds.  The exercises are for both upper and lower body.

I’m reading about similar exercise ideas from Skyler Tanner, Doug McGuff, Nassim Taleb, Jonathan Bailor, and Doug Robb.  Bailor, in his recent book, also recommends only four exercises.  Highcock’s look a little safer for rank beginners. 

The idea is to recruit three different types of muscle fiber during the muscle’s movement.  If you move explosively and finish too soon (get your mind out of the gutter!), you’re only using  one type of muscle fiber (fast twitch, I think).  You want to stimulate a strength and growth response in all three types of muscle fiber.  And explosive or rapid movements are more likely to cause injury, without any benefit. 

Once you get the basic program down, Chris takes you through some easy variations (called progressions) to make the exercises gradually harder, so you continue to improve your strength and fitness. 

Chris understands that many folks can’t do a single push-up.  He takes you through pre-push-up movements to get you prepared  to do actual push-ups.  This goes for all four exercises.  I bet even my little old lady patients could use this program.  (This is not blanket clearance for everybody to use this program; I don’t need the lawsuits.  Get clearance from your own doctor first.)

The exercises incorporate our five basic movements: push, pull, squat, bend/hinge, walk/gait.  The four exercises are: wall sit (squat), push-up, modified row, and hip extension.

My only criticism of the book is that Chris should have used young, attractive, bikini-clad models to illustrate the exercises.  The existing photos are clear and helpful, however.

But seriously, the only suggestion I have for the next version of Hillfit would be to mention that it will take a couple or three weeks to see much, if any, improvement in strength once you start the program.  Same for when you increase the workload with the exercise progressions.  Perhaps this is in there, but I missed it.  You don’t want people quitting in frustration that they’re not seeing progress soon enough.

The author provides scientific references in support of his program, so he didn’t just make this stuff up.  Only one of the references involved mice!

Several “take home” points for me personally are: 1) stretching before or after exercise does nothing to prevent injury or soreness, and may hurt short-term athletic performance, 2) don’t hold your breath, 3) train to “momentary muscular failure.”  I’m not entirely sure what momentary muscular failure means.  It may not be Chris’s term, but it’s prominent in one of his best scientific references.  I use free weights and don’t think I can safely go 100% to momentary muscular failure.  Hitting momentary muscular failure, by the way, is more important than the amount of weight you’re moving.

Aerobic Versus Strength Training for People With Diabetes

“Resistance training, similarly to aerobic training, improves metabolic features and insulin sensitivity and reduces abdominal fat in type 2 diabetic patients,” according to a report in the current issue of Diabetes Care.

Italian researchers randomized 40 type 2 diabetics to follow either an aerobic or strength training program for four months.  The increase in peak oxygen consumption (VO2 peak) was greater in the aerobic group, whereas the strength training group gained more strength.  Hemoglobin A1c was similarly reduced in both groups, about 0.37%.  Body fat content was reduced in both groups, and insulin sensitivity and lean limb mass were similarly increased.  Pancreas beta-cell function didn’t change.

Per this one study, neither type of training seems superior overall.  If you’re just going to do one type of exercise program, choose your goal.  Do you want more strength, or more sustainable “windpower”? 

The Pennington Biomedical Research Center found somewhat different results in their larger and more complex study published in 2010.  However, they were primarily testing for diabetes control (as judged by hemoglobin A1c improvement), rather the improvements in strength or aerobic power.  The found the combination of aerobic and strength training is needed to improve diabetic blood sugar levels.  Both types of exercise—when considered alone—did not improve diabetes control. 

As for me, I do both strength and aerobic training.

By the way, I only read the abstract of the current research, not the full report. High-intensity intervals on a treadmill help me git’r done quicker.

Can Diabetes Be Prevented?

Paula Deen’s recent announcement of her type 2 diabetes got me to thinking about diabetes prevention again.  If you’re at high risk of developing diabetes you can reduce your risk of full-blown type 2 diabetes by 58% with intensive lifestyle modification.  Here’s how it was done in a 2002 study:

The goals for the participants assigned to the intensive lifestyle intervention were to achieve and maintain a weight reduction of at least 7 percent of initial body weight through a healthy low-calorie, low-fat diet and to engage in physical activity of moderate intensity, such as brisk walking, for at least 150 minutes per week. A 16-lesson curriculum covering diet, exercise, and behavior modification was designed to help the participants achieve these goals. The curriculum, taught by case managers on a one-to-one basis during the first 24 weeks after enrollment, was flexible, culturally sensitive, and individualized. Subsequent individual sessions (usually monthly) and group sessions with the case managers were designed to reinforce the behavioral changes.

Although the Diabetes Prevention Program encouraged a low-fat diet, another study from 2008 showed that a low-fat diet did nothing to prevent diabetes in postmenopausal women

I don’t know Paula Deen.  I’ve never watched one of her cooking shows.  She looks overweight and I’d be surprised if she’s had a good exercise routine over the last decade.  I’m sorry she’s part of the diabetes epidemic we have in the U.S.  I wish her well.  Amy Tenderich posted the transcript of her brief interview with Paula, who calculates her sweet tea habit gave her one-and-a-half cups of sugar daily).

  • Nearly 27% of American adults age 65 or older have diabetes (overwhelmingly type 2)
  • Half of Americans 65 and older have prediabetes
  • 11% of U.S. adults (nearly 26 million) have diabetes (overwhelmingly type 2)
  • 35% of adults (79 million) have prediabetes, and most of those affected don’t know it

I think excessive consumption of concentrated sugars and refined carbohydrates contribute to the diabetes epidemic.  Probably more important are overweight, obesity, and physical inactivity.

The Mediterranean diet has also been linked to lower rates of diabetes (and here).  Preliminary studies suggest the Paleo diet may also be preventative (and here).

Greatly reduce your risk of type 2 diabetes by eating right, keeping your weight reasonable, and exercising.

What About “The Biggest Loser”?

Probably not watching The Biggest Loser

Dr. Barry Sears (Ph.D., I think) recently wrote about a lecture he attended by a dietitian affiliated with “The Biggest Loser” TV show.  She revealed the keys to weight-loss success on the show.  Calorie restriction is a major feature, with the typical 300-pounder (136 kg) eating 1,750 calories a day.  On my Advanced Mediterranean Diet, 300-pounders get 2,300 calories (men) or 1,900 calories (women). 

Although not stressed by Dr. Sears, my impression is that contestants exercise a huge amount. 

Go to the Sears link above and you’ll learn that all contestants are paid to participate.  In researching my Conquer Diabetes and Prediabetes book, I learned that the actual Biggest Loser wins $250,000 (USD).  Also, “The Biggest Loser” is an international phenomenon with multiple countries hosting their own versions, with different pay-off amounts.  A former Biggest Loser, Ali Vincent, lives in my part of the world and still has some celebrity status.

This TV show demonstrates that the calories in/calories out theory of body weight still applies.  Including the fact that massive exercise can help significantly with weight loss.  In real-world situations, exercise probably contributes only a small degree to loss of excess weight.  The major take-home point of the show, for me, is that you can indeed make food and physical activity choices that determine your weight.

Most of us watch too much

I know losing 50 to 10o pounds of fat (25–45 kg) and keeping it off for a couple years is hard; most folks can’t do it.  Do you think you’d be more successful if I gave you $250,ooo for your success?

Is Exercise Supposed To Be Fun?

Exercise is not supposed to be fun.  If it is, then you should suspect that something is wrong.

That quote is from an essay by Ken Hutchins posted at the Efficient Exercise website.

When I was a young man in my 30s, I was jogging 20 miles a week and ran a couple marathons (26.2 miles).  I enjoyed it and didn’t do much else for exercise or overall fitness. I thought I was in pretty good shape.  You can get away with that when you’re 35, but not when you’re 50.  At 57 now, I can’t think of any single recreational activity that can help me maintain the overall strength, functionality, and injury resistance I want and need as I age. 

I’ve come to view exercise as a chore, like flossing/brushing teeth, changing the oil in my car, and sleeping when I’d rather not.  I’ve got my current exercise chore whittled down to an hour three times a week.  OK, sometimes just twice a week.

Exercise, Part 11: Target Heart Rate

To get the full health benefits of regular physical activity, you need to put some effort into it.  A leisurely hour-long stroll in the mall while window-shopping doesn’t pass muster, although that’s better than nothing.

One rough way to gauge whether you are working hard enough during aerobic exercise is to monitor your heart rate, also known as pulse.  Subtract your age from 220.  The result is your theoretical maximum heart rate in beats per minute.  Your heart rate goal, or target, during sustained aerobic exercise is a pulse that is 60 to 80 percent of your theoretical maximum pulse.  For example: maximum heart rate for a 40-year-old is 180 (220 – 40 = 180), so the target heart rate zone during exercise is between 108 and 144 (60 to 80 percent of 180).  Exceeding the upper end of the target zone is usually too uncomfortable to be sustainable.  Exercise heart rates below the target zone suggest you’re not working hard enough to reap the full long-term benefits of aerobic exercise.

Here’s how to determine your pulse.  After five or 10 minutes of exercise, stop moving and place the tips of your first two fingers lightly over the pulse spot inside your wrist just below the base of your thumb.  Count the pulsations for 15 seconds and multiply the number by four.  The result is your pulse or heart rate.  It will take some practice to find those pulsations coming from your radial artery.  If you can’t find it, ask a nurse or doctor for help.

Like all rules-of-thumb, this target heart rate zone isn’t always an accurate gauge of cardiovascular workout intensity.  For instance, it is of very little use in people taking drugs called beta blockers, which keep a lid on heart rate.

As you become more fit, you’ll notice that you have to work harder to get your heart rate up to a certain level.  This is a sure sign that your heart and muscles are responding to your challenge.  You may also want to monitor your resting heart rate taken in the morning before you get out of bed. Unfit, sedentary people have resting pulses of 60 to 90.  Athletes are more often in the 40s or low 50s.  Their hearts have become more efficient and just don’t need to beat as often to get the job done.

As you become more fit, you’ll also notice that you have more energy overall and it’s easier to move about and handle physical workloads. You’ll feel more relaxed and have a sense of accomplishment. Expect these benefits eight to 12 weeks after starting a regular exercise program.

Exercise, Part 10: What If I’m Markedly Obese?

The more overweight you are, the harder it will be to exercise.  At some point even light exercise becomes impossible.  Average-height women tipping the scales at about 280 pounds (127 kg) and men at 360 pounds (164 kg) aren’t going to be able to jog around the block, much less run a marathon.  These weights are 100 percent over ideal or healthy levels.  An actual “exercise program” probably won’t be possible until some weight is lost simply through very-low-carb eating, calorie restriction, or bariatric surgery.  The initial exercise goal for you may just be to get moving through activities of daily living and perhaps brief walks and calisthenics while sitting in a chair.

Markedly obese people who aren’t up to the aforementioned extreme weights can usually tolerate a low-intensity physical activity program.  At 50 percent over ideal weight, an average-height woman of 210 pounds (95 kg) is carrying 70 excess pounds (32 kg) of fat.  Her male counter-part lugs around 90 pounds (41 kg) of unnecessary fat.  This weight burden causes dramatic breathlessness and fatigue upon exertion, and makes the joints and muscles more susceptible to aching and injury.  If you’re skinny, just imagine trying to walk or run a mile carrying a standard five-gallon (19 liter) water cooler bottle, which weighs only 43 pounds (19.5 kg) when full.  The burden of excess fat makes it quite difficult to exercise.    

If you’re markedly obese, several tricks will enhance your exercise success.  I want you to avoid injury, frustration, and burn out.  Start with light activity for only 10 or 15 minutes, gradually increase session length (e.g., by two to four minutes every two to four weeks) and increase exercise intensity only after several months.  Your joints and muscles may appreciate easy, low-impact exercises such as stationary cycling, walking, swimming, and pool calisthenics/water aerobics.  You may also benefit from the advice of a personal fitness trainer arranged through a health club, gym, or YMCA/YWCA.  Check out several health clubs before you join.  Some of them are primarily meat markets for beautiful slender yuppies.  You may feel more comfortable in a gym that welcomes and caters to overweight people. Hospitals are increasingly developing fitness centers with obese orthopedic, heart, and diabetic patients in mind.

Recreational Activities Are Not Necessarily Exercise

Exercise is not supposed to be fun.  If it is fun, then you should suspect that something is wrong.

The quote above is from an essay entitled “Exercise vs Recreation” by Ken Hutchins, posted at the Efficient Exercise website.  Skyler Tanner works at Efficient Exercise and his blog is one that I follow.  We have a strange connection.  Skyler grew up in Fountain Hills, AZ; I live about 20 miles from there.  He lives in Austin, TX, now; I lived there for eight years.

Here’s another quote from that essay:

One pound of human fat can support the energy demands of running 35-45 miles, probably more.  This would require the average man to run for 6-8 hours.  He would burn the calories he could easily ingest in as many minutes.

Hutchins’ essay is thought-provoking.  It may change the way you think about exercise.
