Surely placebo is the “gold standard” treatment?
With Placebo, one has only the clinician time cost, and no side-effects.
I suggest that clinicians with a high placebo factor should be able to charge extra for their greater effectiveness.
I addition, those with a nocebo effect should pay a fine.

Physicians do indeed vary in their powers of placebo and nocebo. And big red pills are always more potent than little white ones.
